The SOURCE™ Root Cause Analysis Methodology
You will learn the powerful tools and techniques that are part of the SOURCE™ (Seeking Out the Underlying Root Causes of Events) methodology. The SOURCE methodology is based on tools commonly used throughout industry for incident investigation. ABS Consulting’s unique application of these common tools will guide your investigation, making each investigation efficient and effective. We teach effective interviewing skills along with methods for collecting other types of data. You will learn to analyze data using causal factor charting (event and condition charting), cause and effect tree analysis, and change analysis. The root cause determination techniques include the 5-Whys technique and ABS Consulting's Root Cause Map. Realistic industry examples and workshops are used extensively throughout the course to illustrate key points and to allow you practice your new skills.

Developing Your Program
Besides learning incident investigation skills, you will learn how to fully meet requirements of standards and regulations for investigations of incidents with health, reliability, environment, and quality impacts.
