Course Description
An RCA Methodology Tailored to the Maritime Industry
This course is a special adaptation of our root cause analysis (RCA) and incident investigation techniques for the maritime industry. This course was developed in partnership with American Bureau of Shipping (ABS) and uses their Guidance Notes on the Investigation of Marine Incidents as a reference. We have modified the terminology, examples, and workshops to address the specific issues associated with maritime applications.
Realistic Examples and Workshops
The incident investigation and RCA techniques you will learn in this 2-day course are state of the art. Realistic industry examples, and workshops are used extensively throughout the course to illustrate key points and allow you to practice your new skills.
Learn Our Step-by-Step Approach
The focus is on teaching you how to gather data, analyze data for causal factors, fill gaps in data, determine root causes, and write effective recommendations. You will receive the SOURCE™ Investigator's Toolkit that includes checklists, forms, and guides to help your personnel perform efficient, effective, and consistent investigations. Anyone wishing to lead effective investigations of complex incidents and design a successful RCA program should attend. Take this course with its companion course, Human Error Prevention.
